Preventable adverse events, including medical errors, are a global health challenge. Although the four words are well recognized to every healthcare provider “first, do no harm”, medical errors have still occurred even with the rapid evolution in equipment, technology, and modern medicine.

In 1999, the Institute of Medicine, IOM published the report (To Err Is Human: Building a Safer Health System) which estimated that 44,000 to 98,000 Americans die every year from medical errors. This report generated tremendous public and media attention to how safer the healthcare system is.

Medical errors usually happen not from human mistakes, but from system defects. In this course, participants will be able to develop a system’s view of patient safety challenges and learn strategies about how to improve.
